After care – volumizing consultation
As part of your volumizing consultation at Cosmos Clinic Sydney, the following general after-care instructions will be given to you in person and documented in your medical record. They are provided here for educational purposes only and do not replace personalised medical advice.
Immediate care (first 24 h)
Apply a cool compress for 10 min every hour on the day of treatment to reduce expected swelling and bruising.
Do not apply ice directly; wrap in a clean cloth and avoid firm pressure.
Paracetamol is usually sufficient for tenderness; avoid aspirin and ibuprofen unless already prescribed by your doctor.
Leave the skin untouched—no make-up, perfume or alcohol-based products for 24 h.
Sleep face-up with the head slightly elevated for one night to minimise oedema.
First 48–72 h
Avoid strenuous exercise, saunas, spas, swimming pools and excessive heat.
Gentle facial expressions only—no exaggerated smiling, puckering or chewing gum.
Protect from UV: broad-spectrum SPF 50+ and a wide-brim hat; UV exposure accelerates device breakdown.
Report severe pain, dusky/discoloured skin, rapidly expanding swelling or fever immediately to the 24-hour emergency number provided at discharge.
Ongoing maintenance (weeks to months)

The Fat Transfer to the Buttocks (BBL)is a surgical procedure that involves transferring purified fat from areas such as the abdomen, back, or thighs to the buttocks. What is a Buttock Fat Transfer?

Do I Need to Wear a Compression Garment?
After BBL surgery, wearing a compression garment is highly recommended to assist with swelling management and support the healing process. This garment is tailored to meet the needs of the recovery phase.
You will need to wear the compression garment full-time for the first three weeks, removing it only to shower. After this period, you can reduce wear to part-time, around 12 hours a day, for another three weeks. When removing the garment to shower, it’s best to lie down for 10–15 minutes before standing up to avoid feeling faint. It’s also advisable to keep shower water lukewarm, as excessive heat may impact the fat transfer.
How Should I Sit After Fat Transfer to Buttocks Surgery?
It’s important to avoid putting pressure directly on the treated area during the initial recovery period. For the first two weeks, use a cushion under your thighs to allow your buttocks to remain free of any pressure. A small, firm cushion or a pillow can be helpful for this.
After the first two weeks, you can begin to adjust your sitting position gradually, continuing to minimise pressure on the buttocks for up to six weeks.
What Position Should I Sleep In After Fat Transfer to Buttocks Surgery?
The position you sleep in after surgery is critical. Sleeping on your stomach or at a slight incline (around 45 degrees) is ideal during the recovery period, as it avoids placing pressure on the treated area. If you prefer sleeping on your back, it’s best to wait until 2–3 weeks post-surgery to ensure this doesn’t affect your results.
It’s important to avoid sleeping on your side, as this can reduce circulation to the areas where fat transfer has been performed. Some swelling in the hands, feet, or face may occur in the first few days post-surgery but typically resolves over time.
Are Lymphatic Drainage Massages Necessary?
Lymphatic drainage massages may be advised as part of your post-surgery care. These massages can help reduce swelling, improve comfort, and assist the body’s natural healing process.

In one of our more recent videos posted on the Cosmos Clinic Instagram feed, social media sensation Miss Zedd allows cameras to film her breast augmentation surgery.
Breast fat transfer is an alternative to breast implants for those who desire more volume, greater cup size or enhancement of breast shape. Those best suited for the cosmetic surgery procedure are women who have an excess amount of fat elsewhere in the body that can be removed by liposuction. Most common areas are the abdomen, thighs or back.
During the breast augmentation procedure, the cosmetic or plastic surgeons remove unwanted body fat from the determined areas through Vaser Liposuction. Before the fat grafting takes place, the fat is purified and the healthy fat cells are separated. These cells are used for the fat injection into areas of the breast.
Miss Zedd uses fat transfer to gain volume not bigger breasts
Already blessed with a substantial cup size, Miss Zedd didn’t approach Cosmos Clinic for breast enlargement. Instead, she desired a breast lift and an increase in breast volume in the upper portion of her breasts.
After a successful Brazilian Butt Lift procedure, Miss Zedd is back in the consulting room and lets cameras film her preoperative consultation and breast augmentation surgery.
“The reason for coming in to get a fat transfer is purely to put more volume into the top part of my breasts,” she says. “The lower part is quite full already and that’s not where my issue is. It’s just the fact I don’t have any volume on the top.”
Why fat transfer over breast reconstruction which includes a lift and breast implants?
With a history of poor wound healing and keloid scarring, Miss Zedd was hesitant about undergoing the extensive plastic surgery procedure of a breast lift and implants. “It scared me a bit in the sense that I didn’t want cookie-cutter scaring all over my breasts and with the fat transfer it’s keyhole.”
One of the benefits of using Vaser Liposuction technology is that the entry points are kept small with no need to make large incisions for the device to transfer the injected fat. These small incisions are located as discretely as possible to minimise visible scarring.
Breast augmentation using fat transfer does produce very different results than a breast implant procedure. The finished product will be more natural so it is important to set realistic expectations and discuss those with your surgeon before proceeding.
Follow Miss Zeed into the surgery room

Dr. Francesco Fascetti explains the “aim is to put it [the fat] on those two quadrants, to the top half to try to fill the space.”
In the video, you’ll see Miss Zeed’s cosmetic surgery in progress before she summaries how she was feeling after the surgery. As the procedure negates the need for general anesthesia and is instead performed under twilight sedation, Miss Zeed was only in the treatment room for approximately 1 hour and able to go home the same day.
She will be able to resume normal activities after about 2-5 days and will begin to see the full results of the breast fat transfer once the swelling and bruising subside.

Dr Joseph Ajaka talks with The Morning Show hosts, Kylie Gillies and Larry Emdur, about the rise in popularity of the Brazilian Butt Lift (BBL). The interview was sparked after Kylie and Larry discovered Romanian Instagram influencer Andrea Vasile who makes up to $5,000 per post thanks to her curvaceous derriere.

Kylie continues to explain the BBL cosmetic surgery trend clearly isn’t stopping anytime soon and is largely fueled by celebrities such as Kim Kardashian and Nicki Minaj.

“We have two groups of people who are getting this surgery done – Instagram models who are making a lot of money out of this, like this Romanian influencer, and mums and older women who just want to look better in a tight dress,” explains Dr Joseph.
One of the reasons the BBL surgery is popular among Instagram models is that they’re seeking versatility. In his interview on The Morning Show, Dr Joseph explains there are so many different angles that can be taken of a large, curvaceous buttocks.
“With your breasts, there is only one set of photos you can really take for Instagram. So, it’s all about the different photos you can take with a big bum.”
“The other group of people coming through are the mums and older ladies over 60 who just want to look better in a dress”, explains Dr Joseph.
Women who have smaller, flat bottoms or one that has sagged with age are drawn to the BBL procedure to help them feel feminine in a tight dress. They’re not looking for an outrageously large buttock or one as voluminous Instagram influencer Andrea Vasile. As Dr Joseph explains, they’re just want to feel more confident in dresses.

In The Morning Show interview, Dr Joseph touches on the specifics of the BBL procedure, explaining it is quite involved with patients unable to sit normally for two weeks post-surgery.
The BBL surgery involves taking fat from different areas of the body, including the tummy, love handles, lower back and sometimes even the ankles and calves. After being purified, this fat is this injected into the bum to create the size and shape the patient is desiring.
“So this is win, win”, says Larry.
“Yes, I am loving it so far,” remarks Kylie.

For those wanting to emulate the Kardashian look, they may need more than one session to create such a generous size. This will also depend on the density of your bottom, explains Dr Joseph. For a patient with firm, tight skin, they won’t be able to inject as much fat into the buttocks as someone with thinner, looser skin.
Concluding the interview, Kylie asks for Dr Joseph’s opinion on Instagram Influencers like Andrea Vasile and their exaggerated sized buttocks.
“What I say to my patients is I’m not here to judge you on the shape you want. I’m here to help them achieve the shape if they want to get it, in a way that’s as safe as possible.”

A Cosmetic Surgery Tummy Transformation Too Good Not to Share

A recent and inspiring tummy transformation
Tummy bulge and overhang is a common problem many people experience. It’s also one of the most stubborn areas of excess fat to get rid of naturally and associated with increased health risks. This Cosmos Clinic client came to her cosmetic surgery consultation wanting to look and feel better in clothing as well as improve her overall health.
She opted for Vaser Liposuction to remove the excess fat cells from her upper and lower abdomen. Excess fat was also removed from her franks to help accentuate her natural curve and improve the overall proportion of her figure.
Incredibly, 7 litres of unwanted fat was removed during her cosmetic surgery. This helped to slim her stomach and reduce the size of her overhang.
